1- Providers may make the referral to the office by calling (716) 526-4041. However, it is more efficient if the client calls our office personally. Information is obtained that the provider may not have (credit card information, schedule preference) and it also confirms that the client is ready to assist in their own treatment. Providers may call to alert our office to the referral.
2- Clients may also access our website to request an appointment.
3- Providers may send pertinent information via fax to (716) 526-4161.
4- Parents, care givers and significant others should attend appointments with the client. A legal guardian must be present for clients under the age of 18 years old.
5- Regular attendance for appointments is mandatory. “No show / Missed” appointments are not an acceptable way of sharing in treatment plans. The client must take responsibility for maintaining communication with the provider regarding their health and treatment.
6- Each client will receive a new client packet that includes a medical release of information form to be completed and sent to the referring provider and given to the office so that information regarding treatment can be exchanged.
